What Exercise ACTUALLY Does to Your Hormones (It’s Not What You Think)

Let me tell you about a patient I’ll call Sarah.

Sarah came to see me after three years of trying everything. She was exercising six days a week — long cardio sessions, lots of running, occasionally HIIT classes when she had energy. She was exhausted all the time, her periods were getting more irregular, not less, and her testosterone was creeping up. By every external measure, she was “doing everything right.”

She was not doing everything right. She was doing the right things in the wrong way, for the wrong reasons, without understanding what exercise was actually doing inside her body.

Once we understood the mechanism — and adjusted her approach accordingly — her HOMA-IR improved by 40% in 12 weeks. Her cycle returned. Her energy came back. Same person, same gym membership, completely different outcome.

The mechanism is everything.

How Exercise Talks to Your Cells

Here is the single most important thing I can tell you about exercise and PCOS: muscle contraction is an insulin-independent pathway for glucose disposal.

Let me explain what that means and why it’s revolutionary for PCOS management.

In a normal metabolic state, glucose gets into your cells via GLUT4 transporters — those glucose doors we discussed in Post 1. GLUT4 reaches the cell surface when insulin sends the signal. If your cells are insulin resistant, they ignore the signal, GLUT4 stays inside, glucose stays in the blood.

But here’s the extraordinary thing about muscle: exercise activates GLUT4 translocation through a completely separate pathway — independent of insulin. When a muscle contracts, it activates a protein called AMPK (AMP-activated protein kinase), which signals GLUT4 to move to the cell surface regardless of insulin status.

You don’t need your insulin signaling to work. You don’t need to fix your insulin resistance first. You just need to make your muscles contract. And when they do, glucose floods in.

This is not a small effect. This is the biochemical rationale for why exercise is first-line therapy for insulin resistance. You’re essentially bypassing the broken lock with a key that always works.
