Two Types, Two Different Root Causes
I had back-to-back appointments one Tuesday that perfectly illustrated why PCOS treatment can’t be one-size-fits-all.
10am: Lisa, 33, Type 2 (Inflammatory PCOS)
Lisa walked in exhausted, holding her head. “Another migraine,” she said. “That’s 12 this month.”
Her labs:
Fasting insulin: 9 (perfectly normal)
CRP: 7.8 mg/L (should be <1)
Ferritin: 180 ng/mL (inflammatory marker, should be <150)
Testosterone: elevated
Thyroid antibodies: positive (Hashimoto’s)
She had joint pain, eczema covering her arms, and debilitating fatigue. Her periods came every 45-60 days with severe cramping. She’d tried the “standard PCOS diet” for 6 months with minimal improvement.
11:30am: Emma, 28, Type 3 (Post-Pill PCOS)
Emma looked anxious. “I’ve been off birth control for 14 months and my period still hasn’t come back. And look at my skin—” She gestured to severe cystic acne covering her jawline and cheeks. “I never had acne like this before.”
Her labs:
Fasting insulin: 6 (excellent)
Testosterone: very elevated
LH:FSH ratio: 4:1 (very high)
DHEA-S: elevated
SHBG: crashed (birth control had artificially elevated it)
No inflammatory markers
She’d been on birth control since age 16 (for acne, ironically). Twelve years of synthetic hormones. Now her body had forgotten how to make its own.
Same diagnosis. Completely different problems.
Lisa’s issue was inflammation wreaking havoc on her ovaries. Emma’s issue was hormonal suppression from years of birth control.
They needed opposite approaches.

Understanding Inflammatory PCOS
Here’s what most doctors miss about inflammatory PCOS:
Standard PCOS treatment focuses on insulin. But if your insulin is normal and inflammation is driving your androgens, aggressively lowering carbs or taking metformin won’t help—and might make you feel worse.
The mechanism:
Chronic inflammation → Inflammatory cytokines (IL-6, TNF-α, CRP) → Ovaries produce excess androgens → Disrupted ovulation → PCOS symptoms
It’s not insulin telling your ovaries to make testosterone. It’s inflammation doing it.1
Where does the inflammation come from?
The most common triggers:
Gut dysfunction (leaky gut, SIBO, dysbiosis)
Food sensitivities (gluten, dairy, eggs, soy)
Environmental toxins (pesticides, heavy metals, plastics)
Chronic infections (H. pylori, Epstein-Barr, Lyme)
Autoimmune conditions (Hashimoto’s is common with inflammatory PCOS)
Chronic stress (elevates inflammatory cytokines)
A 2021 study in Reproductive Sciences found that women with inflammatory PCOS (elevated CRP >3) who followed insulin-focused protocols had:2
Minimal improvement in symptoms
Poor ovulation rates (only 18%)
Continued hormonal dysfunction
But women who addressed inflammation first had:
54% reduction in inflammatory markers
61% ovulation rate improvement
Significant symptom resolution
Bottom line: If inflammation is your primary driver, it must be addressed first.
