I want to start this post with something I say to almost every patient I see with PCOS in my practice: you are not lazy, broken, or failing. Your biology is doing exactly what it was designed to do — it’s just doing it in the wrong direction, at the wrong time, in a way that’s working against you rather than for you.
And at the center of that malfunction, in the majority of cases, is a single mechanism that most women with PCOS have never had properly explained to them: insulin resistance.
I’ve been an OB/GYN and reproductive health researcher for years. I’ve sat across from thousands of women who came to me frustrated, confused, and often a little bit heartbroken — because no one had taken the time to explain what was happening in their own bodies. That ends here.
Let’s Start With a Simple Question No One Asks Enough
What does insulin actually do?
Insulin is a peptide hormone secreted by the beta cells of your pancreas. After you eat, carbohydrates break down into glucose, which enters your bloodstream. Your pancreas senses this glucose rise and releases insulin — its job is to escort that glucose out of the blood and into your cells, where it can be used for energy.
Think of insulin as a key, and your cells as houses with locked doors. Insulin pulls up to the door, inserts the key, the lock turns, and glucose flows in. Blood sugar drops. Equilibrium restored. System works beautifully.
Now imagine you’ve been ringing that doorbell so often — sending so much insulin, so frequently, over so many years — that the people inside the house have stopped answering the door. They’re still in there. The key still fits the lock. But they’ve stopped responding. They’ve become resistant to the signal.
That is insulin resistance. Your cells stop listening. Your pancreas, sensing that blood glucose is still too high, does what it’s designed to do: it secretes even MORE insulin. This state — high circulating insulin with cells that ignore it — is called hyperinsulinemia, and it is the engine driving most of your PCOS symptoms.
What Is Actually Happening at the Cellular Level
Here’s the mechanism, made as clear as I can make it:
When insulin binds to a receptor on your cell surface, it triggers a signaling cascade involving a protein called PI3K (phosphatidylinositol 3-kinase), which activates a downstream protein called AKT. That activated AKT then signals vesicles inside the cell containing GLUT4 transporters to migrate to the cell surface.
GLUT4 is the glucose door. When it reaches the cell membrane, glucose can enter. When the cascade is broken — when any step in that chain fails — GLUT4 stays inside the cell, the door never opens, and glucose stays in the blood.
In women with PCOS, research shows this cascade is impaired primarily in skeletal muscle and the liver — the two biggest glucose-clearing tissues in your body. Women with PCOS demonstrate a 27% reduced sensitivity to insulin compared to women without PCOS, even after controlling for BMI. This is not a weight problem. This is a metabolic problem that often comes with a weight problem attached to it.
The Vicious Cycle That’s Keeping You Stuck
Here’s where it gets deeply important to understand, because this is the loop that runs your life with PCOS whether you’re aware of it or not:
Your muscles and liver become insulin resistant → Your pancreas compensates with more insulin → High insulin reaches your ovaries → Your ovaries, critically, remain sensitive to insulin → They respond to the excess insulin by producing excess testosterone → High testosterone increases insulin resistance in muscle → This also suppresses SHBG (sex hormone-binding globulin) in the liver → Lower SHBG means more free testosterone circulating in your blood → More testosterone worsens PCOS symptoms → Symptoms cause stress → Cortisol rises → Cortisol raises blood sugar → More insulin is needed → Cycle repeats.
This is not linear. It’s a loop. You cannot break it by just “eating less” because that only addresses one input into a multidirectional system. Understanding the loop is the first step to interrupting it.
A landmark 2025 review published in the Journal of Clinical Medicine proposed — correctly, in my view — a paradigm shift: PCOS should be understood and managed as an insulin-centric disorder, not primarily a gynecological one. This is something I’ve believed for years based on what I see in clinical practice.
The “Lean PCOS” Conversation We Need to Have
One of the most persistent myths I encounter — even in medical practice — is that insulin resistance only affects women with PCOS who are overweight. This is simply not true, and it causes real harm when lean women with PCOS are dismissed.
Research shows that insulin resistance and hyperinsulinemia occur across ALL weight categories in PCOS. Studies have found that lean women with PCOS show significantly increased glucose-stimulated insulin secretion compared to BMI-matched controls — without detectable insulin resistance on standard tests. This is important: standard fasting glucose and even HbA1c tests can look completely normal while significant insulin dysfunction is already present.
What Tests Should You Actually Be Getting?
This is one of the most actionable things I can give you. If you have PCOS and your doctor has only checked your fasting glucose, you are not getting the full picture. Here’s what to ask for:
The minimum:
Fasting insulin level (not just glucose — insulin is the early warning signal)
HOMA-IR calculation (fasting insulin × fasting glucose ÷ 405). A score above 2.0–2.5 is generally considered insulin resistant; above 2.9 is a stronger indicator
2-hour oral glucose tolerance test (OGTT) with insulin levels — this catches impaired glucose metabolism that fasting tests miss entirely
Fasting glucose AND HbA1c (standard but insufficient on their own)
Fasting lipid panel (insulin resistance alters lipid metabolism significantly)
Consider adding:
Triglyceride-to-HDL ratio (a ratio above 3.0 is a strong clinical marker of insulin resistance and cardiovascular risk)
C-reactive protein (CRP) — a marker of the low-grade inflammation that both drives and is driven by insulin resistance
If your doctor won’t order these, it is completely appropriate to ask specifically for them and explain why you’re concerned. You are your own best advocate.
Why This Matters Beyond Symptoms
I want to be honest with you, because I think honesty is kinder than comfort. Untreated insulin resistance in PCOS is not just about irregular periods and skin issues. The long-term metabolic stakes are significant:
More than half of women with PCOS develop type 2 diabetes by age 40
PCOS significantly increases risk of metabolic syndrome, cardiovascular disease, and non-alcoholic fatty liver disease
Insulin resistance affects endometrial health, oocyte quality, and fertility
Chronic hyperinsulinemia is associated with increased cardiovascular risk independent of weight
This is why I teach this, write about this, and why I built this community. Because PCOS that goes unmanaged is not benign. And PCOS that is well-managed — through informed, targeted, evidence-based strategies — can have dramatically different long-term outcomes.
Your Action Steps This Week
Book an appointment. Ask for fasting insulin, HOMA-IR, and a full metabolic panel. If your current provider won’t order them, consider an endocrinologist or reproductive endocrinologist who has experience managing insulin resistance in PCOS.
Calculate your triglyceride-to-HDL ratio from any recent blood work. Divide your triglycerides by your HDL cholesterol. Above 3.0 warrants attention.
Start tracking your energy after meals. Not calories — energy. Post-meal fatigue, brain fog, and intense sugar cravings within 90-120 minutes of eating are clinical symptoms of dysregulated insulin. Start noticing when they happen and what you ate.
