I want to say something I don’t say lightly, as a physician who is careful about hyperbole: if I could prescribe one intervention — one single thing — to every woman with PCOS I have ever treated, it would be progressive resistance training.
Not a medication. Not a supplement. Lifting weights.
I know that’s a strong statement. Let me back it up.
Muscle Is Not an Aesthetic Organ. It Is a Metabolic Organ.
This is the conceptual shift that changes everything.
Most women think about building muscle in terms of how they’ll look. That’s fine. But from a metabolic medicine standpoint, muscle is something entirely different: it is the largest insulin-sensitive tissue in your body, responsible for approximately 80% of post-meal glucose disposal.
Say that again: eighty percent of the glucose that enters your bloodstream after a meal is cleared by your skeletal muscle. Not your liver. Not your fat cells. Your muscles.
Now think about what happens when you have insufficient muscle mass — whether from sedentary living, undereating, or chronic cardio without resistance training. You have less tissue available to clear glucose. Blood sugar stays elevated longer. Insulin levels stay higher. The PCOS loop tightens.
Every pound of muscle you build is not just a cosmetic achievement. It is a literal expansion of your metabolic capacity. You are physically building more glucose-clearing infrastructure.
